

Position Overview:
Are you a compassionate and organized individual with a desire to make a direct impact on youth and families? Step's Foundation, Inc. is seeking a dedicated Case Management Intern to join our team. This internship offers a unique opportunity to gain hands-on experience in client advocacy, resource coordination, and case management within a nonprofit setting.
In this role, you will work closely with our peer educators, program assistants, and advocacy and outreach team to provide comprehensive support services for students and their families. We take a "warm hand-off" approach, meaning we don't just refer families to external services; we stay involved to ensure they successfully connect with the resources they need and track the outcomes of these connections.
Key Responsibilities:

Client Support: Assist with intake assessments, gathering background information, and identifying client needs and goals.

Resource Coordination: Help connect students and families with community services such as housing, food assistance, healthcare, and mental health support.

Warm Hand-Offs: Actively participate in guiding families through the referral process, including initiating contact with service providers and providing follow-up support to ensure successful connections.

Tracking and Reporting: Maintain accurate case notes and records of referrals, documenting outcomes and client progress to support ongoing service planning.

Team Collaboration: Work closely with internal teams, including peer educators and outreach staff, to develop individualized service plans and address barriers to care.

Community Engagement: Support outreach efforts to strengthen partnerships with local service providers and community organizations.
Qualifications:

Currently enrolled in a degree program in social work, psychology, human services, or a related field.

Strong interpersonal and communication skills, with a genuine passion for helping others.

Excellent organizational skills and attention to detail.

Ability to manage sensitive information and maintain confidentiality.

Familiarity with local community resources and social service systems is a plus.
